This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.<br><br><ul><li>You must live in and be licensed in Connecticut and you will work remotely from CT. <br><br></li></ul>Location<br><br>Remote in Connecticut<br><br><strong>Job Description<br><br></strong><ul><li> Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth</li><li> Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians</li><li> Partner with our neurologist(s) and broader care team to ensure continuity of care</li><li> Educate patients and families on diagnoses, treatment plans, and disease progression</li><li> Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements<br><br></li></ul><strong>Key Accountabilities<br><br></strong><ul><li> Active, unrestricted NP license in Connecticut; certification in geriatrics, adult-gerontology, or family practice preferred</li><li> 2+ years of clinical experience in neurology, memory care, and/or dementia care settings</li><li> Strong clinical judgement and clinical documentation skills</li><li> Comfortable managing a full patient panel independently with physician collaboration</li><li> Excellent communication skills with patients facing cognitive decline and their caregivers<br><br></li></ul><strong>The Local Infusion Way<br><br></strong>Local Infusion is a respectful upbeat team united by our mission of shaping the way specialty infusion care is delivered.
