<strong>The position is open to candidates located in CO and NM. </strong><br><br>This job can be on-site or from home. If you work from home, this position runs software that <strong>requires high-speed internet with 25 MB download and 15 MB upload speeds.</strong> The ability to consistently run this software is a requirement to be successful in this position. If you work from home, you will need a quiet space that is free of distractions, so that you can focus on your job responsibilities. Background noise should be limited, and you must be able to maintain a space that provides patient confidentiality. You may be required to be in the office occasionally for administrative duties, training, or teambuilding.<br><br><strong>Supervisor:</strong> Virtual Care Center Manager, Telehealth<br><br><strong>Percent On-Site vs Remote: </strong>90% Remote/ 10% onsite (You may be required to be in the office occasionally for administrative duties, training, or team building)<br><br><strong><strong>Schedule: </strong></strong>Full Time ∼37.5 hours/week, Benefits Eligible, Non-Exempt<br><br>Rotating Shift Monday - Sunday <br><br><strong>Virtual Care Center Hours:</strong><br>Monday- Friday: 8 am-7 pm<br>Saturday: 8am-3 pm (when assigned)<br>Sunday: Closed-However, rotating assignment to cover the PPDirect App and respond to patients asynchronously 3 times a day.<br><br><strong>Position Salary Range:</strong> $50.05/hour - $63.41/hour<br><strong>Starting Pay:</strong> Although our full pay range is included above, new hires are typically brought in at a salary range of $50.05/hour - $56.73/hour<br><br><strong>The Objective:</strong> To provide educational, supportive, and empathetic patient care and quality assurance in collaboration with PPRM protocols, the medical director, and state and federal regulations.<br><br><strong>Success: </strong>You will know you are successful when all patients who receive follow-up care or telehealth services through the Virtual Care Center (VCC) have their needs met in a patient-centered manner and in accordance with PPRM medical policies.<br><br><strong>Snapshot: </strong>Much of your time will be spent on the computer in a virtual environment providing care to diagnose and treat issues and conditions related to reproductive, sexual health, abortion, and gender-affirming care. Much of your time will be spent using Electronic Health Records and the Electronic Practice Management system, documenting patient interactions, and running reports. Some of your time will be spent interacting with team members and collaborating with other departments. You may work from home or the office. You may be required to be in the office occasionally for administrative duties, training, or team building.<br><br><strong>Qualifications:</strong><br><br><ul><li>Certified Nurse Practitioner (NP) by a national certification organization.</li><li>Must reside in and be able to obtain an RN license in the states of Colorado, New Mexico, and Wyoming.</li><li>Have worked one or more years in sexual, reproductive health, or gender-affirming care.</li><li>You must have active licensure in Colorado, New Mexico, Nevada or Wyoming and must have, or be willing to obtain, controlled substance prescriptive authority in Colorado, New Mexico, Nevada, and Wyoming.</li><li>You are experienced in providing empathetic, patient-centered care.</li><li>You are detail-oriented and experienced in multitasking, meeting deadlines, and prioritizing work independently with little supervision.</li><li>You have strong computer skills and the ability to navigate all Microsoft Suite applications.</li><li>You must be willing to apply for and maintain licensure in other states.</li><li>You have no restrictions on your ability to be credentialed with Medicaid or other payors.</li></ul><br><br><strong>Please complete this </strong><strong>APC Pre-Hire Skills Self-Assessment Checklist</strong><strong> along with this application. </strong><br><br><strong>Key Approaches to the Work:</strong> <br><br>Strong communication<br><br>Strong Operator<br><br>Flexible<br><br>Equity and Inclusion Champion<br><br><strong>Essential Functions:</strong><br><br>Provide Outstanding Patient Care: At its core, your work is about providing care to patients who need you and doing so with the highest degree of competence and quality. To do this you will:<br><br><ul><li>Triage medical questions and concerns from patients, including providing daytime Call for Abortion patients in accordance with PPRM's Medical Standards and Guidelines (MS&Gs).</li><li>Provide asynchronous care for patients seeking birth control, emergency contraception, and/or UTI treatment by the PPDirect app, which is available for patients 7 days a week.</li><li>Conduct patient visits via phone or video to diagnose and treat issues and conditions related to reproductive, sexual health, abortion, and gender-affirming care.</li><li>Perform and order diagnostic studies as indicated and provide appropriate treatment and follow-up as necessary.</li><li>Answer patient questions received via the Patient Portal.</li><li>Provide excellent customer service to both internal and external customers by working towards a positive outcome with any problems encountered.</li><li>Run reports, and document accurately, appropriately, and efficiently all phone interactions with patients in their medical chart in Electronic Health Records (EHR) and Electronic Plan Management (EPM) systems.</li><li>Assist with scheduling patient appointments as needed.</li><li>Perform mandatory STI reporting to Colorado, Nevada, New Mexico, and Wyoming State Departments of Health, when indicated.</li><li>Support Capacity-Building Efforts Internally: You will work to build capacity in others who see patients, serving as a leader with expertise and knowledge that can support others.
